Puerto Rico Hospitals Face Financing and Workforce Challenges

Puerto Rico's hospital sector is confronting a combination of financing pressures, workforce challenges, demographic shifts and technological changes that could reshape how healthcare is delivered across the island, industry leaders said during the 2026 convention of the Puerto Rico Hospital Association. The three-day event brought together hospital executives, government officials, academics, insurers and healthcare experts from Puerto Rico and the United States to examine issues affecting the financial sustainability and operational capacity of the island's healthcare system.

Central concerns included the future of Medicare and Medicaid funding, the changing demographics of Puerto Rico, workforce needs, and the ability of hospitals to maintain services while adapting to new models of care. Jaime Plá Cortés, president and chief executive of the Puerto Rico Hospital Association, said decisions being made in Puerto Rico and Washington could directly affect hospitals' ability to provide services and respond to patients' needs.

The association announced the launch of a study examining the socioeconomic contribution of Puerto Rico's hospital system, expected to provide data on the sector's broader impact on the island's economy. Workforce development emerged as another key issue as healthcare organizations assess the talent needed to support changes in medical care, technology and hospital operations. Discussions also addressed financing models, risk management and strategies to strengthen long-term sustainability.

The sector is also preparing for changes in demand driven by demographic, social, economic and climate-related factors.
